Affidavit of Retail Labor Rate

To renew your Warranty Labor Rate please complete this short online Affidavit.

In addition to the online application below, you will need to email or fax the following 3 items below in order to complete the submission of your application.

1) Photograph of posted retail labor rate

2) 5 copies of retail customer paid repair bills reflecting the posted rate.

3) Copies of the most recent Indmar Training Certificates of all Certified Technicians employed by your dealership.

Thank you for submitting your application and thank you for choosing Indmar! Do not forget that you application is NOT complete until we we receive the required documentation. (details above) Upon approval, our Customer Service team will provide your dealership with an Indmar Dealership Number and a Warranty Labor Rate. Your Dealer Number will be established in our accounting system along with your Warranty Labor Rate. We will also provide your dealership with a current Indmar Dealer Operations Manual and a Flat Rate Guide.
